Проблема тяжелой сочетанной травмы является одной из актуальных проблем современной медицины. Лечение пострадавших с тяжелой сочетанной травмой проводится с постоянным наблюдением за тяжестью их общего состояния. В настоящее время отсутствует единый подход к решению проблемы оценки тяжести травм.</p><p>Цель обзора — осветить состояние проблемы объективной оценки тяжести травм,а также возможность и особенности применения различных методов оценки тяжеститравм при сочетанной черепно-лицевой травме.</p></sec><sec><title>Методы</title><p>Методы. Проведен анализ зарубежных и отечественных научных работ по теме исследования, опубликованных за последние 10 лет, с использованием поисковых систем «PubMed» и «Научная Электронная Библиотека». Поиск проводился с применением следующих ключевых слов: «тяжесть черепно-лицевой травмы», «тяжесть травм», «шкалы оценки тяжести травм», «trauma severity», «trauma severity score», «injury severity», «cranio-facial trauma severity». Отбор научных работ проведен в зависимости от их научной ценности относительно темы исследования.</p></sec><sec><title>Результаты</title><p>Результаты. Основу систематического обзора составляют данные 49 научных работ по исследуемой теме. В статье рассмотрены наиболее известные в мире методы оценки тяжести травм, доступные для применения при оценке тяжести сочетанной черепно-лицевой травмы. В зависимости от принципа оценки тяжести травм рассмотренные методы разделены на 3 группы: анатомические, физиологические и комбинированные. Освещены исторические аспекты создания рассмотренных балльных методов, основные их преимущества и недостатки. Обсуждены особенности применения рассмотренных методов у пострадавших с сочетанной черепно-лицевой травмой. Подробно описаны шкалы оценки тяжести изолированной травмы челюстно-лицевой области.</p></sec><sec><title>Заключение</title><p>Заключение. На сегодняшний день в клинической практике отсутствует общепринятая система оценки тяжести травм, в том числе черепно-лицевой травмы. Большинство предложенных методов оценки тяжести травм предназначены для решения задачи прогнозирования исходов травмы. В то же время динамическое наблюдение за общим состоянием пострадавших на госпитальном этапе оказания медицинской помощи производится чаще всего с использованием неспецифичных методов оценки тяжести общего состояния.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Severe combined trauma is a pressing issue in modern medicine. Victims with a severe combined trauma receive constant monitoring for the severity of their condition. There is no commonly adopted uniform model for assessing the severity of injuries.</p></sec><sec><title>Objectives</title><p>Objectives. To review existing scoring methods for assessing the severity of combined craniofacial trauma.</p></sec><sec><title>Мethods</title><p>Мethods. A search of Russian and foreign publications in the PubMed and Elibrary databases at the depth of 10 years was conducted. The query terms were: injury severity, trauma severity [тяжесть травм], trauma severity score [шкалы оценки тяжести травм], cranio-facial trauma severity [тяжесть черепно-лицевой травмы]. The record selection was based on its scientifi c value in this research topic.</p></sec><sec><title>Results</title><p>Results. This systematic review covered 49 scientifi c papers reporting methods for assessing the severity of combined craniofacial trauma. Depending on the main applied principle, the severity scoring methods were classifi ed into 3 groups: anatomical, physiological and combined. Along with the history of creation, main advantages and disadvantages of the methods in terms of scoring performance in combined craniofacial trauma were outlined. Severity scoring models in isolated maxillofacial trauma were described in detail.</p></sec><sec><title>Conclusion</title><p>Conclusion. There is no generally accepted best clinical practice for trauma severity scoring, including craniofacial trauma. The majority of scoring models are developed for survival chance estimation.
