Обменная нефропатия приобретает все большую распространенность, на долю которой, по данным источников литературы разных авторов, приходится от 27 до 64% в структуре нефрологической патологии детского населения. В связи с этим проблема правильной постановки диагноза, назначение диетической и медикаментозной терапии имеют особую значимость.</p><p>Цель обзора — предоставить информацию последних лет о методах лабораторной и инструментальной диагностики и тактике лечения детей с обменными нефропатиями.</p></sec><sec><title>Методы</title><p>Методы. Проведен анализ российской и зарубежной литературы в базах РИНЦ, ВАК, Scopus, Web of Science, Pubmed, The Cochrane Library, eLIBRARY. Глубина поиска по временному параметру ограничивалась семью годами. В качестве маркеров поиска были использованы такие ключевые слова, как «exchange nephropathy», «children», «calcium oxalate crystalluria», «therapy», «обменная нефропатия», «дети», «оксалатно-кальциевая кристаллурия», «терапия». Методы исследования, которые были использованы в процессе анализа: контент-анализ, описательно-аналитический.</p></sec><sec><title>Результаты</title><p>Результаты. В данном обзоре было проанализировано 74 источника литературы. Для дальнейшего анализа было отобрано 50 статей.</p></sec><sec><title>Обсуждение</title><p>Обсуждение. Приведены современные представления об обменных нефропатиях: классификация, особенности клинического течения, методы диагностики, лечения и профилактики. В наши дни наблюдается тенденция в сторону более раннего возраста заболеваемости обменными нефропатиями. Наиболее изученной из всех обменных нефропатий является так называемая обменная нефропатия с оксалатно-кальциевой кристаллурией. В статье также показана роль такого микроорганизма, как Oxalobacter formigenes, в развитии вторичной оксалатной нефропатии.</p></sec><sec><title>Заключение</title><p>Заключение. Лечение обменных нефропатий как полиэтиологических заболеваний представляет собой комплекс, который должен воздействовать и на причинный фактор, и на механизмы, способствующие процессу литогенеза. В статье на основании анализа данных литературы представлены современные подходы к терапии обменных нефропатий у детей.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Metabolic diseases are growing, also in paediatric practice. Metabolic nephropathy is becoming more prevalent comprising 27–64% total paediatric nephrological cases, according to various published evidence. In this concern, the correct diagnosis, dietary and drug therapy acquire particular importance.</p></sec><sec><title>Objectives</title><p>Objectives. An overview of current methods for laboratory and instrumental diagnosis and treatment of metabolic nephropathies in children.</p></sec><sec><title>Methods</title><p>Methods. National and foreign literature sources were mined in the RSCI, VAK, Scopus, Web of Science, PubMed, Cochrane Library and eLibrary databases at a search depth limited to seven years. The query keywords were: metabolic nephropathy [обменная нефропатия], children [дети], calcium oxalate crystalluria [оксалатно-кальциевая кристаллурия], therapy [терапия]. Content and descriptive analyses were used as research tools.</p></sec><sec><title>Results</title><p>Results. The review surveyed 74 literature sources, with 50 selected for further analysis.</p></sec><sec><title>Discussion</title><p>Discussion. Modern views on metabolic nephropathies are highlighted in terms of classification, clinical progression, methods of diagnosis, treatment and prevention. A current trend in these diseases is earlier-age morbidity. The most studied metabolic nephropathy is calcium oxalate crystalluria. The article also discusses the role of Oxalobacter formigenes bacteria in secondary oxalate nephropathy.</p></sec><sec><title>Conclusion</title><p>Conclusion. Therapies for metabolic nephropathies as polyaetiologic diseases are multicomponent to target both a causal factor and lithogenesis-contributing mechanisms.
