Болезнь Паркинсона — нейродегенеративное заболевание, приводящее к тяжелой инвалидизации, особенно на поздних стадиях. Медицинская помощь должна быть выполнена согласно современным клиническим рекомендациям и стандартам. Оценка качества оказания медицинской помощи в Российской Федерации проводится на основании изучения медицинской документации экспертами страховых компаний на соответствие ее российским стандартам и другим регламентирующим документам.</p><p>Цель исследования — изучить предикторы неблагоприятного течения болезни Паркинсона по результатам плановых экспертиз качества медицинской помощи.</p></sec><sec><title>Методы</title><p>Методы. Проведено ретроспективное когортное исследование 7264 экспертиз качества медицинской помощи 1754 пациентов с болезнью Паркинсона за период с 2011 по 2018 год на территории Российской Федерации. Были изучены два признака: наличие/отсутствие прогрессирования заболевания и определение/усугубление группы инвалидности или сохранение прежней степени социальной и профессиональной активности. Полученные данные были использованы для построения диагностических таблиц прогностических факторов течения болезни Паркинсона по А. Вальду. Статистическая обработка данных проводилась с использованием пакета прикладных статистических программ Statistica 13 (StatSoft, США).</p></sec><sec><title>Результаты</title><p>Результаты. Разработаны диагностические таблицы для определения вероятных факторов, влияющих на прогрессирование болезни Паркинсона и соответственно ухудшение степени социальной и/или профессиональной активности с оценкой диагностических коэффициентов, при достижении которых риск ухудшения состояния пациентов с болезнью Паркинсона составит более 95%.</p></sec><sec><title>Заключение</title><p>Заключение. Наибольшее прогрессирование болезни Паркинсона (p &lt; 0,05) был отмечено в тех случаях, где были выявлены дефекты сбора жалоб, анамнеза и клинической оценки состояния пациентов, неадекватное назначение или вообще отсутствие назначения препаратов леводопы при наличии показаний, а также дефекты коррекции фоновых заболеваний. Вероятность появления или усугубления социальной дезадаптации пациентов с болезнью Паркинсона (p &lt; 0,05) была отмечена чаще в тех случаях, когда обнаружены дефекты сбора жалоб и анамнеза, неврологического осмотра и клинической оценки состояния пациентов, а также неадекватного назначения или вообще отсутствие назначения препаратов леводопы при наличии показаний.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Parkinson's disease is a neurodegenerative disorder that leads to severe disability, especially in its advanced stages. Medical care should be performed in accordance with modern clinical recommendations and standards. Assessment of the quality of medical care in the Russian Federation is carried out via examination of medical documentation by insurance experts regarding compliance with Russian standards and other regulatory documents.</p></sec><sec><title>Objective</title><p>Objective. To study the predictors for unfavorable course of Parkinson's disease based on routine examinations of the quality of medical care.</p></sec><sec><title>Methods</title><p>Methods. The cohort retrospective study included 7,264 examinations of the medical care quality of 1,754 patients with Parkinson's disease for the period from 2011 to 2018 in the Russian Federation. The study focused on two indicators: presence/absence of disease progression and determination/aggravation of a degree of disability or maintenance of the previous social and professional activity. The obtained data were used to develop diagnostic tables with prognostic factors of Parkinson's disease according to A. Wald. Statistical analysis of the results was carried out using Statistica 13 (StatSoft, USA). Results. Diagnostic tables have been developed to determine the prognostic factors affecting the progression of Parkinson's disease and, accordingly, decreasing the degree of social and/ or professional activity and to assess the diagnostic factors when the risk of the disease aggravation will be more than 95%.</p></sec><sec><title>Conclusion</title><p>Conclusion. The greatest progression of Parkinson's disease (p &lt; 0.05) was observed in the cases of poor collection of complaints, history and clinical evaluation of the patient's condition, inadequate prescription or absence of prescription of levodopa when indicated, as well as defects in the correction of additional disorders.
