Имеющиеся сравнительные исследования, рассматривающие вопрос выбора схемы назначения антибиотика при дентальной имплантации, зачастую носят исключительно наблюдательный характер, заключающийся в описании тех признаков в послеоперационном периоде, которые врач может увидеть. Безусловно, для более объективной оценки течения послеоперационного периода и сравнения подходов требуются лабораторные исследования. Одним из доступных на стоматологическом приеме методов исследования является изучение биохимических свойств слюны.</p><p>Цель исследования — сравнительная оценка однократной и пролонгированной схем приема амоксициллина/клавуланата после дентальной имплантации по показателям неспецифического иммунитета в смешанной слюне.</p></sec><sec><title>Методы</title><p>Методы. Проведено рандомизированное неконтролируемое исследование 30 пациентов с диагнозом частичной адентии. Клинический этап исследования выполнен на базе кафедры пропедевтики хирургической стоматологии федерального государственного бюджетного образовательного учреждения высшего образования «Московский государственный медико-стоматологический университет имени А. И. Евдокимова» Министерства здравоохранения Российской Федерации, лабораторный этап — на кафедрах фармакологии и биологической химии в той же организации, в 2021–2022 гг. В исследование включали пациентов в возрасте от 18 до 75 лет при условии благоприятных анатомических условий для установки дентального имплантата. До исследования не были допущены пациенты с соматическими заболеваниями в стадии декомпенсации. Пациентов случайным образом распределили в 2 группы. 1-й группе было показано за 30 минут до операции принять таблетированную форму амоксициллина/клавуланата 875+125 мг. 2-й группе за 30 минут до операции было показано принять амоксициллин/клавуланат 875+125 мг, а затем в течение 7 дней после операции принимать по 1 таблетке амоксициллина/клавуланата 500+125 мг 2 раза в день. Для оценки эффективности применяемой схемы антибиотикотерапии у всех пациентов натощак производился сбор нестимулированной смешанной слюны не менее 1 мл до операции и на 3-и сутки после операции. В образцах слюны спектрофотометрическим методом определяли количество иммуноглобулинов классов G, M, А (мг/л) и активность α-амилазы (МЕ/л). Статистическая обработка лабораторных данных проводилась с использованием t-критерия Стьюдента с поправкой Уэлча с использованием программы Statistica для Windows 10 Pro (StatSoft, США). Межгрупповые и внутригрупповые отличия считали достоверными при р ≤ 0,05.</p></sec><sec><title>Результаты</title><p>Результаты. Среди изученных белков слюны наиболее выраженная разница между группами пациентов была получена по показателю IgA. По другим исследованным показателям слюны достоверная разница между группами не была получена на 3-и сутки после операции. При этом, несмотря на отсутствие статистической разницы, по всем показателям отмечается тенденция к более сильному росту количества IgА, G, IgM, активности амилазы слюны, а также отмечена высокая корреляционная взаимосвязь между иммуноглобулинами классов A, G; классов G, M, а также классов A, M.</p></sec><sec><title>Заключение</title><p>Заключение. У пациентов пролонгированной схемы приема такие показатели слюны, как иммуноглобулины G и M, активность слюнной α-амилазы, не показали достоверных различий между группами, однако показали тенденцию к более сильному увеличению указанных иммуноглобулинов в слюне пациентов 1-й группы. Наиболее значимая разница была получена по показателю в слюне иммуноглобулина A, количество которого значимо меньше было в группе пролонгированного приема антибиотика (p &lt; 0,05).</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. The available comparative studies on issues of choosing antibiotic regimen in dental implantation are often observational and describe only visible postoperative signs. More objective assessment of postoperative period and comparison of approaches requires laboratory tests. One of the methods to be used at a dental appointment is to study examination of the biochemical properties of saliva.</p></sec><sec><title>Objective</title><p>Objective. To carry out comparative evaluation of single and prolonged regimens of amoxicillin/clavulanate administration after dental implantation indicated by nonspecific immunity in mixed saliva.</p></sec><sec><title>Methods</title><p>Methods. A randomized uncontrolled trial of 30 partially edentulous patients was conducted. The clinical phase of the study was performed at the Department of Propaedeutics of Surgical Dentistry, MSUMD; the laboratory phase — at the Pharmacology and Biological Chemistry Departments, MSUMD, in 2021–2022. The study included patients aged 18 to 75 years, anatomically suitable for dental implant placement. Patients with somatic diseases in the decompensation stage were excluded from the study. Patients were randomized into 2 groups. The group 1 was prescribed to take amoxicillin/clavulanate pills (875+125 mg) 30 minutes before surgery. The group 2 was to take amoxicillin/clavulanate (875+125 mg) 30 minutes before surgery and then 1 pill of Amoxicillin/ Clavulanate (500+125 mg) 2 times a day for 7 days after surgery. To evaluate the efficacy of the applied antibiotic therapy, unstimulated mixed saliva was collected from all patients on an empty stomach, 1 ml before surgery and on the third day after surgery. The amount of immunoglobulins of G, M, A classes (mg/l) and б-amylase activity (IU/l) were determined spectrophotometrically in the saliva samples. Statistical processing of laboratory data was performed using Welch's t-test by means of Statistica 10 Pro (StatSoft, USA). Intergroup and intragroup differences were considered significant at p ≤ 0.05.</p></sec><sec><title>Results</title><p>Results. Based on the saliva proteins examined, the most pronounced difference between the patient groups was obtained for IgA. For other studied parameters of saliva, no significant difference between the groups was obtained on the third day after the surgery. However, despite the absence of a statistical difference, a tendency to a stronger increase in the amount of IgA, G, IgM, salivary amylase activity was noted for all indicators; moreover, a high correlation between immunoglobulins of class A, G, immunoglobulins of class G, M, and immunoglobulins of class A, M was revealed.</p></sec><sec><title>Conclusion</title><p>Conclusion. Among patients with the prolonged regimen, the groups did not differ significantly in such saliva parameters as immunoglobulins G and M and salivary б-amylase activity, however, a tendency to a stronger increase of the mentioned immunoglobulins in the saliva of group 1 patients was revealed.
