Кавернозные мальформации могут сочетаться с различными заболеваниями центральной нервной системы, в том числе с опухолями головного мозга. В научной литературе встречаются немногочисленные описания сочетания кавернозных мальформаций и менингиом. При сочетании кавернозной мальформации и опухоли возможны различные варианты течения заболеваний, при этом симптомы могут манифестировать в разной последовательности через многие годы. Представленный случай описывает выявление у пациентки множественных кавернозных мальформаций ствола головного мозга и правой подкорковой области de novo спустя 14 лет после тотального удаления менингиомы задней трети большого серповидного отростка.</p><p>Описание клинического случая. Пациентка 55 лет поступила в порядке перевода из государственного бюджетного учреждения здравоохранения «Абинская центральная районная больница» Министерства здравоохранения Краснодарского края в государственное бюджетное учреждение здравоохранения «Научно-исследовательский институт — Краевая клиническая больница № 1 имени профессора С. В. Очаповского» Министерства здравоохранения Краснодарского края с жалобами на головокружение, неустойчивость при ходьбе, нарушение речи и глотания, тошноту. Из анамнеза известно, что 14.02.2008 была успешно оперирована по поводу менингиомы задней трети большого серповидного отростка. В течение 5 лет регулярно наблюдалась нейрохирургом консультативно-диагностической поликлиники государственного бюджетного учреждения здравоохранения «Научно-исследовательский институт — Краевая клиническая больница № 1 имени профессора С.В. Очаповского» Министерства здравоохранения Краснодарского края. Признаков продолженного роста опухоли и другой патологии головного мозга не выявлялось, в связи с чем пациентка прекратила посещать поликлинику. Спустя 14 лет внезапно развились головокружение, неустойчивость при ходьбе, нарушение речи и глотания, тошнота. При обследовании у пациентки были выявлены кавернозные мальформации ствола головного мозга и правой подкорковой области. Стволовая кавернозная мальформация манифестировала кровоизлиянием в мозг. Наличие выраженного неврологического дефицита являлось показанием к оперативному лечению. Выполнена срединная субокципитальная краниотомия, удаление кавернозной мальформации ствола. На фоне лечения неврологический дефицит регрессировал.</p></sec><sec><title>Заключение</title><p>Заключение. Сочетание множественных кавернозных мальформаций и менингиом встречается редко. Оперативное лечение следует проводить в зависимости от превалирующей в данный момент патологии. Удаление кавернозной мальформации с ствола головного мозга может улучшить функциональные результаты лечения.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Cerebral cavernous malformations may coexist with various disorders of the central nervous system, including brain tumors. There are few descriptions of combined cavernous malformations and meningiomas in the literature. When a cavernous malformation co-occurs with a tumor, the diseases may have various progression trajectories, and symptoms may appear in different sequences over many years. The present case describes the detection of multiple de novo cavernous malformations of the brainstem and the right subcortical region 14 years after the total resection of a meningioma arising from the posterior third of the falx cerebri.</p></sec><sec><title>Case description</title><p>Case description. A 55-year-old woman was transferred from the Abinsk Central District Hospital to the Research Institute — Prof. S.V. Ochapovsky Regional Hospital No. 1 with complaints of dizziness, gait instability, dysarthria, dysphagia, and nausea. According to her history, on 14 February 2008 she had undergone successful resection of a meningioma of the posterior third of the falx cerebri. For five years she was regularly followed by a neurosurgeon at the consultative and diagnostic center of Prof. S.V. Ochapovsky Regional Hospital No. 1; no evidence of tumor re-growth or other intracranial pathology was found, after which she discontinued follow-up. Fourteen years later she developed sudden-onset dizziness, gait instability, speech and swallowing disturbances, and nausea. Examination revealed cavernous malformations of the brainstem and right subcortical region. The brainstem cavernous malformation had presented with intracerebral hemorrhage. The pronounced neurological deficits were an indication for surgical intervention. A midline suboccipital craniotomy with resection of the brainstem cavernous malformation was performed. Neurological deficits regressed during postoperative management.</p></sec><sec><title>Conclusion</title><p>Conclusion. The combination of multiple cavernous malformations and meningiomas is rare. Surgical treatment should be guided by the pathology that is clinically dominant at the time.
