Интерлейкин-1β играет важную роль в патогенезе коронавирусной инфекции (COVID-19), а также может быть информативен в качестве прогностического маркера инфекции. Другие широко используемые диагностические и прогностические маркеры — С-реактивный белок и ферритин.</p></sec><sec><title>Цель исследования</title><p>Цель исследования: оценка влияния транскраниальной электростимуляции на концентрацию интерлейкина-1β, С-реактивного белка, ферритина в крови у пациентов с COVID-19 среднетяжелого и тяжелого течения, не получающих таргетной противоцитокиновой терапии.</p></sec><sec><title>Методы</title><p>Методы. Рандомизированное проспективное исследование охватило пациентов, проходивших лечение в государственном бюджетном учреждении здравоохранения «Краевая клиническая больница № 2» Министерства здравоохранения Краснодарского края в период с 24.06.2021 по 23.02.2022. Пациенты с диагнозом новой коронавирусной инфекции (COVID-19) среднетяжелого и тяжелого течения разделены на группу сравнения (n = 20), получавшую стандартное лечение в соответствии с действующими методическими рекомендациями, и группу транскраниальной электростимуляции (n = 15), в которой к стандартной терапии дополнительно применялась транскраниальная электростимуляция (один сеанс в день до 7-го дня госпитализации в COVID-отделение). Из исследования исключались пациенты, получавшие специфическую антицитокиновую терапию (моноклональные антитела, киназные ингибиторы, рекомбинантные антагонисты рецепторов цитокинов). Уровни IL-1β, С-реактивного белка и ферритина оценивали до начала лечения и по завершении первой недели терапии. Статистический анализ и визуализация данных выполнены в среде R (The R Foundation, Австрия). Различия считались статистически значимыми при p &lt; 0,05.</p></sec><sec><title>Результаты</title><p>Результаты. До начала лечения уровни IL-1β, С-реактивного белка и ферритина между группами статистически значимо не различались. К концу первой недели в группе сравнения уровень IL-1β снизился на 11,8% (p = 0,7), что не являлось статистически значимым. В группе транскраниальной электростимуляции наблюдалось достоверное снижение концентрации IL-1β на 71% от исходного уровня (p = 0,007). К тому же концентрация IL-1β в группе транскраниальной электростимуляции была на 52% ниже, чем в группе сравнения (p = 0,005). Уровень С-реактивного белка значимо снизился в обеих группах, тогда как уровень ферритина статистически значимо не изменился.</p></sec><sec><title>Заключение</title><p>Заключение. Включение транскраниальной электростимуляции в комплексное лечение пациентов с COVID-19 среднетяжелого и тяжелого течения способствует более выраженному снижению сывороточной концентрации интерлейкина-1β по сравнению со стандартной терапией, что может свидетельствовать о противовоспалительном потенциале данного метода.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Interleukin-1β plays an important role in the pathogenesis of coronavirus disease (COVID-19) and may also be informative as a prognostic marker of infection. Other widely used diagnostic and prognostic markers include C-reactive protein and ferritin.</p></sec><sec><title>Objective</title><p>Objective. To evaluate the effect of transcranial electrical stimulation on the serum levels of interleukin-1β, C-reactive protein, and ferritin in moderate to severe COVID-19 patients who do not receive targeted anti-cytokine therapy.</p></sec><sec><title>Methods</title><p>Methods. The conducted randomized prospective study included patients treated at the Regional Clinical Hospital No. 2 (Ministry of Health of the Krasnodar Krai) in the period from June 24, 2021 to February 23, 2022. The patients diagnosed with moderate to severe COVID-19 were divided into two groups: comparison group (n = 20) receiving standard treatment as per the current guidelines and a group (n = 15) in which, in addition to standard therapy, the patients received transcranial electrical stimulation (one session per day until day 7 of stay in the COVID ward). The study excluded patients receiving specific anti-cytokine therapy (monoclonal antibodies, kinase inhibitors, and recombinant cytokine receptor antagonists). The levels of interleukin-1β, C-reactive protein, and ferritin were assessed prior to treatment and at the end of the first week of therapy. Statistical analysis and data visualization were performed in the R environment (The R Foundation, Austria). Differences were considered statistically significant at p &lt; 0.05.</p></sec><sec><title>Results</title><p>Results. Prior to treatment, no statistically significant differences were observed in the levels of interleukin-1β, C-reactive protein, and ferritin between the groups. By the end of the first week, the level of interleukin-1β decreased by 11.8% (p = 0.7) in the comparison group, which was not statistically significant. In the group receiving transcranial electrical stimulation, interleukin-1β levels exhibited a significant decrease (by 71%) from the baseline (p = 0.007). In addition, interleukin-1β levels in this group were 52% lower than in the comparison group (p = 0.005). Also, C-reactive protein levels decreased significantly in both groups, while ferritin levels did not exhibit any statistically significant changes.</p></sec><sec><title>Conclusion</title><p>Conclusion.
