Описанный более полувека назад противоточно-множительный механизм движения мочи (уродинамики) в петле Генле является гипотетическим и предполагает, что уродинамика в канальцевой системе нефрона осуществляется под влиянием физико-химических процессов, которые до настоящего времени не представлены в количественном анализе.</p></sec><sec><title>Цель исследования</title><p>Цель исследования: разработать модель уродинамики в канальцевой системе нефрона и установить числовой критерий, уменьшение которого (с переходом через единицу — пороговое значение) характеризует патологическое замедление уродинамики (уростаз) в петле Генле, где в последующем возникает энергетическая нагрузка на эпителиальные клетки.</p></sec><sec><title>Методы</title><p>Методы. Использованы методы математического моделирования, основанные на учете особенностей мочетока в канальцевой системе нефрона, где отсутствует гладкомышечный аппарат, а непосредственная уродинамика обеспечивается за счет осмотического (электрохимического) эффекта.</p></sec><sec><title>Результаты</title><p>Результаты. Данные результатов анализа позволяют оценить возможность количественной (численной, цифровой…) оценки пропускной способности петли Генле — это проблема, которая давно привлекала внимание специалистов, большинство которых исповедуют гипотетический противоточно-множительный механизм, осуществляемый в восходящем отделе («колене») петли Генле, где предполагается пассивное (без энергетических затрат на выведение Na+) из просвета канальца в интерстиций с целью сохранения продольного осмотического градиента, благодаря которому осуществляется уродинамика в канальцевой системе нефрона.</p></sec><sec><title>Заключение</title><p>Заключение. Вывод, который можно сделать на основе полученного результата, состоит в том, что при различных причинах, опосредованно замедляющих уродинамику в канальцевой системе нефрона (лимфостаз, флебостаз…), пассивное выведение Na+ из восходящего отдела петли Генле затрудняется, что способствует энергетической активизации в эпителиальных клетках на этом участке, то есть увеличивается факультативная продукция АТФ для активного выведения Na+ из просвета канальца в интерстиций. При хронизации энергетической нагрузки (катаболизма) в эпителиальных клетках петли Генле происходит перестройка митохондриального биогенеза и возникают условия для формирования в базальном «лабиринте» кальций-фосфатных (СаР) агломератов.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Described more than half a century ago, the countercurrent multiplication mechanism in the loop of Henle remains hypothetical; it suggests that tubular flow is significantly influenced by physicochemical processes that have yet to be quantified. Objective: To develop a model of tubular fluid dynamics and establish a numerical criterion, where a decrease below unity (threshold value) indicates pathological flow deceleration (tubular stasis) in the loop of Henle, leading to subsequent metabolic stress on the epithelial cells.</p></sec><sec><title>Methods</title><p>Methods. The study used mathematical modeling to account for specific flow characteristics within nephron tubules; given the absence of smooth muscle, tubular fluid dynamics are driven primarily by osmotic (electrochemical) effects.</p></sec><sec><title>Results</title><p>Results. The findings provide a means to quantify (numerically or computationally) the flow capacity of the loop of Henle. This issue has long attracted the attention of specialists, most of whom support the hypothetical countercurrent multiplication in the ascending limb. This mechanism assumes passive Na+ efflux from the tubular lumen to the interstitium to maintain the longitudinal osmotic gradient, which drives the tubular fluid dynamics.</p></sec><sec><title>Conclusion</title><p>Conclusion. The results suggest that various factors indirectly slowing the tubular fluid dynamics (such as lymphostasis and venous stasis) impede the passive Na+ efflux from the ascending limb. This triggers metabolic activation in the epithelial cells in this region, increasing the facultative production of adenosine triphosphate to drive active lumen-to-interstitium Na+ efflux.
