Клинический опыт использования флекаинида в нашей стране пока ограничен в связи с относительно недавней регистрацией препарата в Российской Федерации. Необходимо составить представление о безопасности и переносимости данного препарата при его назначении для профилактики эпизодов фибрилляции предсердий на современном этапе применения, обобщив новые данные.</p></sec><sec><title>Цель исследования</title><p>Цель исследования: на основании данных опубликованных рандомизированных контролируемых исследований оценить безопасность и переносимость приема флекаинида при его назначении для профилактики эпизодов фибрилляции предсердий взрослым пациентам.</p></sec><sec><title>Методы</title><p>Методы. Проведен систематический поиск рандомизированных контролируемых исследований по протоколу PRISMA в базах данных PubMed (MEDLINE, PreMEDLINE, NLM), Cochrane Library, период публикации с января 2015 по март 2026 г., в которых препарат флекаинид назначался взрослым пациентам для долгосрочного контроля ритма при фибрилляции предсердий. В метаанализ для получения обобщенной величины эффекта включены значения отношения рисков для конечной точки безопасности — risk ratio (RR) с 95% доверительным интервалом.</p></sec><sec><title>Результаты</title><p>Результаты. 933 статьи прошли проверку, в итоге для данного обзора было отобрано пять исследований (1265 пациентов). В двух из пяти включенных в метаанализ рандомизированных контролируемых исследованиях присутствовало отдельное выделение первичной точки безопасности и нежелательных эффектов. В трех исследованиях авторы объединяли в показатели безопасности нежелательные явления, связанные с приемом флекаинида, включая проаритмогенный эффект. На основании анализа, проведенного по модели случайных эффектов с применением метода обратной дисперсии для сравнения отношения рисков, статистически значимых различий в точках безопасности между группой пациентов, получавших флекаинид, и контролем не обнаружено. Относительный риск (RR) составил 0,69 с 95% доверительным интервалом 0,36–1,31.</p></sec><sec><title>Заключение</title><p>Заключение. Результаты проведенного метаанализа показали относительную безопасность применения флекаинида при правильном отборе пациентов и контроле проаритмогенных рисков. В то же время полученные результаты следует интерпретировать с осторожностью из-за ряда ограничений, а также необходимости индивидуального подхода для пациентов с высокой коморбидностью в реальной клинической практике. </p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. In the Russian Federation, clinical experience with flecainide remains limited due to its relatively recent regulatory approval. Therefore, it is essential to evaluate the current safety and tolerability of this drug for the prevention of atrial fibrillation by synthesizing recent data.</p></sec><sec><title> Objective</title><p> Objective. To evaluate the safety and tolerability of flecainide for the prevention of atrial fibrillation in adult patients using data from published randomized controlled trials.</p></sec><sec><title>Methods</title><p>Methods. A systematic search of the PubMed (MEDLINE, PreMEDLINE, NLM) and Cochrane Library databases was conducted following the PRISMA guidelines for studies published between January 2015 and March 2026. This review included randomized controlled trials evaluating flecainide administration in adult patients for long-term rhythm control of atrial fibrillation. To obtain a pooled effect size, the meta-analysis incorporated risk ratio values with 95% confidence intervals for the safety outcomes.</p></sec><sec><title>Results</title><p>Results. A total of 933 articles were screened, resulting in the inclusion of five studies (1265 patients). Two of the five randomized controlled trials included in the meta-analysis separately evaluated the primary safety outcome and adverse events. In the remaining three studies, the authors reported flecainide-related adverse events, including proarrhythmia, as part of safety outcomes. Based on a random-effects model using the inverse variance method to compare risk ratios, no statistically significant differences in safety outcomes were found between the flecainide-treated group and the control group. The risk ratio was 0.69, with a 95% confidence interval of 0.36 to 1.31.</p></sec><sec><title>Conclusion</title><p>Conclusion. The results of this meta-analysis demonstrate the relative safety of flecainide when patients are appropriately selected and proarrhythmic risks are carefully monitored.
