Abstract
We have completed 78 operations about giant hernias of the anterior abdominal wall using the new method of hernioplasty. Modeling of direct muscle sheaths was performed using a mesh endoprosthesis. We have compared of sizes (frontal, sagittal and cranio-caudal) and the volume of the abdominal cavity before and after operation according to computed tomography. The obtained results show that the recommended method hernioplasty does not reduce the volume of the abdominal cavity and may be considered a rational surgery with giant hernias.

Abstract
The article presents the results of the comparative analysis of maternal and fetal thrombophilia in patients with early (under 34 weeks of gestation) and late forms of pre-eclampsia. The diagnosing of latent acquired and genetic thrombophilia in women of high risk groups makes possible to predict the risk of pre-eclampsia and to substantiate the prophylaxis of complicated pregnancy. Such prophylaxis reduces the rate and the heaviness of pre-eclampsia and the number of perinatal complications.

Abstract
Using the method of differential thermometry of biologically active points assessed the effectiveness of the treatment in 2 groups of patients in the acute period of ischemic stroke. The first group of patients received a standard set of allopathic medicines for the treatment of cerebral infarction. Patients of the second group together with standard pharmacotherapy took homeopathic medicine arnica C6 per os. The results of the study confirmed objective and subjective data at the beginning and at the end of the course of rehabilitation treatment. Was also conducted correlation analysis revealed a correlation between the indicators of temperature difference and indicators of recovery of neurological deficit, psycho-emotional status and quality of life of patients in the acute period of ischemic stroke, as well as comparative analysis of changes in the temperature difference in the two groups studied in different periods of observation.

Abstract
The aim of this study was to investigate the antibacterial effect of bacteriotoxical light-therapy on root canal dentine. The microbial colonisation of root canal dentine can lead to failures in conventional endodontic treatment if only an inadequate bacterial reduction is achieved through canal treatment and chemical disinfection. The new opportunities present the possibility of using bacteriotoxical light-therapy in treatment of periodontitis.

Abstract
In this paper we propose a selection criterion that is applicable for the treatment of patients with dental arch asymmetry caused by loss of a premolar on one side of the mouth. According to a given criterion, the correlation between tooth size and dental arch parameters is determined. When applying this criterion for finding the correlation, the frontal distal diagonal size of the dental arch was multiplied by 1.14 coefficient. The sum of the mesio-distal diameters of seven teeth in the half-arc was subtracted from the obtained value. The value which was equal to 0±1.0 mm indicated the correlation between tooth size and dental arch parameters. We can conclude if there is correlation between tooth size and dental arch parameters, it is advisable to provide treatment associated with having the post-extraction socket opened and implant therapy performed (most commonly with the use of intraosseous dental implants).

Abstract
Shunt surgery does not have any alternative in refractory glaucoma treatment. The results of this surgical direction are restrained by high risks of intra- and postoperative complications. Considerable fluctuation of intraocular pressure and inflammatory process relate to intraoperative complications. This problem is partly solved by use of viscoelastic fluids nevertheless we do not have precise ideas about qualities of viscoelastic fluids which would be appropriate in this surgery. The comparative study between three groups of viscoelastic fluids in vitro to invextigate the adhesive capacity of leucocytes was carried out. The provocation of adhesive molecules exspression on the leucocytes membranes in experiment was estimated as reaction that activates inflammatory process in the conditions of surgical trauma. Sodium hyaluronate 3% was determined to possess the best qualities. This preparation activate the leucocytes adhesion to a lesser degree, and is capable to be in the anterior chamber for a long time. Use of sodium hyaluronate 3% prevents rough scarring by decreasing inflammation rate.

Abstract
The specific features of epidural anesthesia and an early postoperative period were studied in gynecological patients with different tolerance to transient hypoxia and hypercapnia. The relationship of low tolerance to transient hypoxia and hypercapnia and instable hemodynamics as vasoplegia, which was a risk factor of perioperative complications, including postoperative nausea and vomiting, was proven.

Abstract
The paper deals with the results of cytochemical studies of biologically active enzyme systems of neutrophilic granulocytes and their comparative analysis to assess the effectiveness of gingiva preparation and scalpel use for the correction of the gingival margin. A detailed study of biologically active substances in dynamics in the peripheral blood neutrophils with differentiated definition of cell populations showed no clear difference of cell diversity and excess of content and activity of enzymes of oxidoreductases and hydrolases groups in comparing the effectiveness of gingiva preparation or scalpel use. Asynchronous oscillations of hydrolase activity at gingiva preparation cause a decrease of metabolic processes in the field of surgery.

Increase of number of children having chronic pathology in a stage of a decompensation and children with functional health deviations is revealed. Abstract
In the article reported the results of analysis of the treatment of 531 patients with fracture of the proximal humeral. As part of the study it was shown that using of the external osteosyntesis provides good results, which are not depend on bone mineral density. The results of the conservative treatment depend on premorbid condition of the osseous tissue. Whence external osteosyntesis is more convenient, the method of intramedullary fixation provides better results, and conservative treatment is reasonable for elder year class with normal bone mineral density.

Abstract
Scientific studies and clinical observations confirm that human dental arch forms reveal a great variety. There has been an analysis conducted aiming at detecting the interrelation between sagittal and transversal sizes of maxillary dental arches in 287 patients (both sexes; in their early adulthood, aged 21-35) with physiological occlusion of permanent teeth. Three types of maxillary dental arches were proposed: vestibular dental arch, alveolar lingual (palatal) arch, and dentoalveolar arch. There has been an interrelation established between the sagittal (the depth of the arch) and transversal (the width of the arch) sizes of the maxillary dental arches, and to evaluate the dental arch form it has been recommended to determine the arch index (the ratio between its depth and width). With the dental arch index of 0.74±0.03 the arch form was defined as mesognathic; the dentoalveolar arch and alveolar arch index then was 0.9±0.05. In case of a dental arch index below 0.7, the dentoalveolar arch and alveolar arch index being less than 0.85, the arch form was viewed as brachygnathic. The form was classified as dolichognathic in case the index went beyond 0.77 with the dentoalveolar arch and alveolar arch indices exceeding 0.95.

Abstract
Professional athletes represent a unique group of dermatological patients, since practically all of them, over the whole length of their sport career, encounter dermatological problems. It is known that in the case of origination of dermatological diseases, mainly of bacterial and fungal nature, the sport results level of professional athletes is known to decrease. In this connection, the need arises in a well-timed diagnosis of the athletes' skin microbiocenosis condition. We have improved a method that enables to determine the quantitative and qualitative composition of the mostly represented groups of the microbial population inhabiting the skin.

Abstract
In the article the results of studying the quality of the life of patients with tuberculosis are represented. By the authors were interviewed 500 patients GBUz SK «Boundary antitubercular dispensary» (g. Stavropol'). Is reliably established reduction in the quality of the life of TB patients, which was manifested in a constant fatigue; the constantly decreased mood; reduction in the ability to work; worsening in the sexual life; worsening in the nourishment; reduction in the incomes; the appearance of family problems and the disintegration of family. Is established the connection between the health of patients and the form of their illness, and also other social and economic factors. Conclusion about the need for considering the obtained facts during the organization of medico-social aid to patients with tuberculosis is made.

Abstract
The possibility of vardenafil using for pharmacological preconditioning in surgery is studied. Experiment was carried out on Wistar rats. The influence of vardenafil on the survival of pedicled isolated skin flap and the state of the microvasculature in the ischemic skeletal muscle was defined in comparison with the effect of distant ischemic preconditioning. It was revealed that vardenafil improves survival of pedicled isolated skin flap and stimulates angiogenesis in the ischemic leg muscles of rat.

Abstract
In given article the clinical effect of the complex approach to a choice of tactics of medical-surgical algorithm at patients with a widespread purulent peritonitis is analyzed. Abstract
Acellular dermal matrix (ADM) is known as an innovative material used for breast reconstruction using tissue expanders or implants. Using ADM provides additional coverage and support to the lower pole of the reconstructed breast. In the process of the present study evaluated the risk of postoperative complications when using ADM in the reconstruction based on the implant. Methods We performed a retrospective analysis of 212 consecutive immediate breast reconstruction with tissue expanders or implants, including without ADM 108 and ADM 104. Total was performed breast reconstruction in 140 patients, including 68 unilateral reconstructions and 72 bilateral reconstructions. In the group where unused ADM was higher seroma frequency 13.5% vs. 3.7%, and a higher incidence of infection 18.3% vs. 5.6%. Also, found that body mass index, a statistically significant factor for the development of seroma and infection. Conclusions ADM is a convenient material for breast reconstruction, but it is accompanied with complications such as seroma and infection. Improve the results of operations for breast reconstruction using ADM help the careful selection of patients, the choice of an adequate size of the implant and the proper postoperative management.

Abstract
Tumors of the maxillofacial region are complicated by the problem of prosthetic dentistry. The complexity of the location, diversity, localization, fast growth, high morbidity and risk of progression and recurrence are the reason for late diagnosis and volumetric destructive interventions. In this regard, all the more there is a need for non-invasive diagnosis in the dental admission for additional early diagnosis, monitoring and determination of the effect of treatment of this category of patients.

Abstract
The authors of this article have studied 200 jaw bone CT scans of fully and partially edentulous patients of retirement age. The study identified the minimum, maximum and average values of residual alveolar ridge separately for men and women. The conclusions were made about the possibility of installing dental implants in those patients without using bone grafting. The most favorable conditions for that were found in male patients. According to the study, the most optimal parameters of residual alveolar ridge for the installation of screw dental implants, found on mandible.

Abstract
In the experiment on myocardial infarction (MI), pathophysiological aspects and heart remodeling processes was carried out. It was shown that VEGF and FGF is the most promising angiogenic factors in MI. It has been shown that the use of multipotent mesenchymal stem cells is an effective method of influence on regulation of angiogenesis by increasing the angiogenic factors. Therapeutic effect of multipotent mesenchymal stem cells is achieved due to the stimulation of neoangiogenesis, increasing of the number of vessels and the diminishment of the area of the scar. Our data are consistent with the world literature that can be recommended to use of this type of cellular cardiomyoplasty in the future clinic research.

Abstract
The purpose of this study was to analyse the prerequisites for individual approach to infusion therapy after major abdominal surgery by identifying safe boundaries of water balance in intra and postoperative period, taking of the direct current potential in preoperative period. A retrospective study of perioperative was performed on 396 patients. Revealed that the level of the direct current potential allows to select patients with risk of postoperative complication, based on excessive water balance.

Abstract
The article analyzes the parameters of cardiorespiratory synchronism (CRS) depending on the tonus of the autonomic nervous system in order to evaluate the role of the vegetative index of Kerdo as a factor-argument to improve the method of the diagnosis of the human body's regulatory and adaptive capacities. Existing step by step method for the finding of the minimum bound of the synchronization range can be improved by the forecast of the minimum bound that is necessary for tests with the given breathing frequency. Comparative analysis of CRS-parameters in the examinees with sympathicotonia and vagotonia identified the significant disparity between the value of «the difference between initial heart rate and the minimum bound» (in the cardiac cycles). The link between the autonomic nervous system and the minimum bound of the synchronization range will reduce the number of tests for evaluation of the minimum bound, especially in the examinees with vagotonia for which “the difference between initial heart rate and the minimum bound” is bigger in 3.5 times.

Abstract
To evaluate informational content of sonography for diagnosing of adenomyosis and endomyometritis in case of diffuse changes in myometrium 142 fertile patients who undergone transvaginal sonography with Doppler, dilatation and curettage with 4-foci loop biopsy of myometrium or zug-biopsy of endometrium under hysteroscopy control were examined. The control group consisted of 22 patients with male factor infertility. For differential evaluation of ultrasound markers of adenomyosis and endomyometritis discriminate function analysis was used. For further evaluation of predictive potential of the resulting discriminant function the data of ultrasound examination of 86 patients of prospective group were processed. Sonography with the use of the resulting discriminant function has allowed to diagnose adenomyosis with sensibility 96,4%; specificity 91,7%; likelihood ratio of positive result (LR+) 11,61; of negative result (LR-) 0,04; predictive validity of positive test (Р+) 0,964; of negative test (Р-) 0,917. For endomyometritis diagnosing sensibility amounted to 93.9%; specificity - 91,7%; LR+ - 11,31; LR- - 0,07; Р+ - 0,939; Р- - 0,917.

Abstract
The research of 51 doctors opinions from different regions of the Russian Federation and the near abroad (Kyrgyzstan) with respect to prophylactic salpingectomy. This study demonstrated the awareness of doctors regarding the current views on prophylactic salpingectomy. According to the results of the main part of the questionnaire 35 of 51 (68.6%) responded that they made bilateral salpingectomy when performing a hysterectomy without oophorectomy for benign disease: 22 of 35 (62.9%) to reduce the risk of developing cancer, 16 of 35 (45.7%) to reduce the risk of formation of hydrosalpinx, 6 of 35 (17.1%) in order to reduce the risk of pelvic pain and 5 doctors (14.3%) to reduce the risk of reoperation. Some questions from the main part of the questionnaire suggests selecting one or more answers.

The hypertrophic osteoarthropathy (HOA) may occur as isolated finding or is often part of the syndrome which is characterized by periostosis of the long bones and occasional painful joint enlargement. The majority of cases of HOA are due to malignant thoracic tumors. The authors present a case of hypertrophic osteoarthropathy as a paraneoplastic syndrome in a patient with peripheral lung cancer.

Abstract
The aim of the study was to develop a laboratory risk assessment methodology caries recurrence during pregnancy on the contents of the antimicrobial factor in innate immunity in oral fluid. Surveyed 207 pregnant women with caries of the teeth (clinical group), 31 a pregnant woman with no dental pathology and 32 healthy donors without physical and dental diseases. The patients in oral fluid determined three immune peptides: lactoferrin, cathelicidin LL-37 and α-defensin. In pregnant women the development of dental caries has been accompanied by increased in 2 trimester on trimester 1 level all immune peptides in saliva with the subsequent reduction in antimicrobial peptides α-defensin1-3 and cathelicidin LL37 in oral fluid in the 3 trimester. A compensatory mechanism for constraining the increase of intensity and activity of caries, the recurrence of the disease in pregnancy is increase antibacterial protein lactoferrin in saliva. In pregnant women during the first trimester of gestation when the oral fluid levels of lactoferrin above 3,08 g/ml, lowering the content of cathelicidin is below 1,9 pg/ml, caries recurrence risk goes up.

Abstract
The aim - the evaluation of Selivanov surgery efficacy by the analysis of circulatory shifts in the terminal part of rectum by sonographic data after different interventions for chronic hemorrhoids. The study includes 86 patients who were divided into 3 groups: I - after operation by Milligan-Morgan (30), II - after operation by Ferguson (30), III - after operation by Selivanov (26). Group IV - control (10) - healthy people. All the patients underwent endorectal ultrasonic examination. One demonstrated that in 14 days in the group III the velocity of arterial blood flow and diameter of hemorrhoid arteries was evidently less than before surgery, and approach to paramenters of group IV (respectively 13,2±0,5 vs 11,9±0,8 cm/ sec and 1,18±0,3 vs 0,92±0,08 mm). At the same time in group III one notified evident decrease of thickness of epithelialsubepithelial layer thickness in the left and back-right segment (respectively down to 2,1±0,9 and 2,8±0,4 mm). It testifies to high radicalism of Selivanov surgery and simultaneously to high potency of vascular relationship restoration.

Abstract
The study presents data about structural interaction between decellularizedrat heart matrix and stem cells used for recellularization. The effect of decellularized scaffold influence on the potential cells ability for differentiation without growth factors was evaluated. Morphological analysis of recellularized rat heart matrix was conducted. Rat heart scaffold influence on the proliferative cells capacity during recellularization process was assessed by immunohistochemical analysis. Cell viability on the scaffold was determined by the colorimetric test with MTT-reagent.

Abstract
Features of clinical displays at sick by a salmonellosis with suspicion on surgical diseases and tactics of introduction of such patients are studied. At 13 patients with the confirmed salmonellosis (with gastrointestinal the form-10, generalized the form-3) preliminary hospitalized the analysis of the case record is lead to a surgical hospital with suspicion on various surgical diseases. Carried out researches have established complexity of differential diagnostics at a primary stage in communication with similarity of clinical displays of illnesses. And only dynamic supervision over patients, and in some cases necessity of surgical intervention allowed to specify the diagnosis.

Abstract
In survey there has been done the evaluation of the prolonged remissions of alcohol withdrawal syndrome pattern. The individual biological and personal characteristics impact on quality and dynamic of the pathopsychological disorders has been described. The factors predicted the prolonged remissions have been studied. The persistent stabilization process of pathopsychological disorders in remission has been concluded to be dependent on the biological faults of alcohol exchange. The active therapeutic treatment for about a year after the alcohol refusal including drug therapy and alcohol exchange methods is necessary to form prolonged remissions. Taking into account all above mentioned the necessity to find out the markers of degree of the specific biological damages and the ways of their adjustment determines the importance of the scientific search.
