Abstract
The department of coloproctology Mikaelian Institute of surgery operated on 142 patients with cancer of the distal rectum. In addition to the standard abdominal extirpation of the rectum (n=65) performed sphincter-preserving (n=41), reconstructive (n=16) and plastic (n=20) operations. Postoperative mortality was 2,1%. All postoperative complications were divided into two groups: general surgical nature and due to the technical features of the surgery of the distal rectum. In the immediate postoperative complications and were reported in 18 (12,6%) patients (10 men and 8 women). All of these patients had comorbidities. Among patients with complications prevailed patients (44,4%) with deep intramural spread (рТ4) of the tumor.

Abstract
The department of coloproctology «Mikaelian Institute of surgery» for the last 13 years, operated 941 patient about colorectal cancer. In 381 (40,5%) patients malignant tumor was localized in the rectum, in 142 (37,3%) of them, it had a distal location. Quality of life was studied in 65 patients with cancer of the distal rectum. To assess the quality of life used test version of the questionnaire SF-36 Health Status Survey. The study showed that in the preoperative period in patients with cancer of the distal rectum is a reduction of all the parameters of quality of life, and in the immediate and late postoperative period the quality of life is determined by the type and volume of surgical intervention. Significantly better quality of life after sphincter-preserving and reconstructive-plastic surgery.

Abstract
Research of 42 patients with the closed changes of a diaphysis of a clavicle to which at a nakostny osteosynthesis plates in 22 cases on the top surface of a clavicle were established, and in 20 - on a forward surface is carried out. The rentgenografiya method is used, and also the analysis of value judgment is carried out by the patients of own condition. The plate arrangement on a forward surface of a clavicle led to decrease in number of complications, increase of stability and durability of fixing of a change.

Abstract
The aim of this work is to study changes in functional activity of blood monocytes in patients with chronic hepatitis C virus (HCV), depending on the dynamics of gender-specific treatment. Our studies have shown that monocytes of men with chronic hepatitis C before treatment of moderate activity observed voltage diaphorase and esterase activity, and LDH, with the oppression of LDH activity and G-6-PDG. In women, however, a sharp voltage spectrum of enzymatic test monocytes. The use of combined therapy (with the addition of cycloferon) in men caused a complete normalization of enzymatic spectrum (both quantitative and qualitative). Women have completely returned to normal activity esterase and diaphorases.

Abstract
In this paper we have carried out the diagnosis of the damage of the vascular endothelium in patients with ischemic heart disease (IHD) based on the study of gene of endothelial nitric oxide synthase polymorphism, the condition of vasoregulating function of endothelium, endothelin-1 and the level of type C of natriuretic peptide. 30 patients with IHD and 27 somatically healthy individuals of the Astrakhan region as a control group have been examined. Patients with IHD demonstrated the development of endothelial dysfunction and the prevalence of 4a/4b genotype of endothelial nitric oxide synthase. We have revealed the influence of VNTR-polymorphism on the production of type C of natriuretic peptide and endothelin-1 in ischemic heart disease.

Abstract
In this research, the authors established the role of fractalkine as a marker of inflammatory activity at concomitant respiratory cardiac pathology, which flows with potentization of systemic inflammatory reactions. We found that in patients with arterial hypertension (AH), and in patients with chronic obstructive pulmonary disease (COPD) activation of systemic inflammation is occured, more expressed in patients with COPD. At the same time, the development of AH in patients with COPD is not accompanied by increasing of fractalkine levels, in comparison with mononozology. When combined COPD with coronary heart disease (CHD) has increased production of fractalkine compared with mononozology (COPD, CHD). The combination of COPD+CHD more adversely in relation to the systemic inflammatory activation.

Abstract
In assessing of polymorphonuclear leukocytes at diabetes of I type by children is fixed the decrease of their bactericidal activity with a deficit of absorption, secretion of active oxygen radicals, functional reserve. It is shown the increase of the readness to apoptosis, that was accompanied by increasing of the expression of CD95, decreasing - Bcl2. The increase of the cytotoxic potential of neutrophils in the form of the increasing level of myeloperoxidase and lysosomal cationic proteins is revealed.

Abstract
To estimate a functional condition of a myocardium of the left ventricle we examined 36 patients with the moderate course of bronchial asthma (BA), 39 patients with the heavy course of the disease and 30 somatic healthy patients. According to the results of echokardioskopic examination of the patients of both groups with BA we revealed the diastolic dysfunction of the left ventricle which intensified in the process of the disease increasing. According to lengthening of time of isovolumetric contraction of the left ventricle and increasing in a Doppler echocardiographic index - the Tei index - we revealed the destruction of diastolic function of the left ventricle and moderate impairment of systolic function at patients with BA of a heavy persistent current of the disease.

Abstract
100 patients with CHF functional class (FC) I against the background of hypertensive disease of stage II with regular left ventricular (LV) ejection fraction (EF) (LVEF≥55%) took part in the research and were randomized into two groups. Group 1 consisted of 51 patients (54,2±22,9 years old) and received metoprolol succinate (Betalok ZOK, «AstraZeneca», Sweden) in a dosage of 78,1±19,3 mg per day. Group 2 consisted of 49 patients (55,4±23,1 years old) and received Quinapril (Accupro, «Pfizer», USA) in a dosage of 17,3± 7,7 mg per day. Initially and 6 months into therapy, the following tests were done: VO2max treadmill test, echocardiography, 6-minute walk test, NT-proBNP test. In order to objectively quantitatively determine regulatory adaptive status (RAS), a cardiorespiratory synchronism test was done. Results of the research showed that both medications improved indices of diastolic function of the LV, however only Quinapril effectively changed its structural and geometric parameters and systolic function. Exercise tolerance increased, NT-proBNP level decreased, VO2max increased, RAS improved only with Quinapril treatment. Therefore Quinapril has the advantage over metoprolol succinate in patients with CHF FC I and regular LVEF against the background of HD of stage II.

Abstract
This article discusses the gender differences study results up to the frequency of occurrence of the most difficult diagnosed caries process located in the area between the contacts. The men and women chewing teeth thorough inspection, using visual and tactile clinical examination and inter-proximal radiography, was conducted, that is the most accurate method at the diagnostics of teeth contact surfaces caries (CSC). Dental caries prevalence analysis for chewing teeth contact surfaces was held out, both in general and separately by teeth type (molars and premolars), on the upper and lower jaws, on the right and left sides. The trends and statistically significant lateral teeth gender frequency of occurrence differences (FOD) according to the groups of dental row, and teeth type were identified.

Abstract
A study was conducted of the results of endoscopic surgery for De Quervain's disease in 68 patients in the period between 2000 and 2012. We recommend endoscopic method that provides a complete audit of the primary and secondary channels cut retinaculum and decompression tendons. Endoscopic surgery allowed to reach 97% of successful results without relapse. In 66 of 68 patients with de Quervain's disease , after endoscopic release possible to reach full volume extension and flexion without pain. Our results indicate that endoscopic release De Quervain 's disease is a successful operation.

Abstract
A study was conducted of the results of endoscopic surgery for Dupuytren's disease in 14 patients in the period between 2005 and 2012. Dupuytren's disease in patients with diabetes differs from palmar fibromatosis patients without diabetes. Painful nodules and scar ray palmar fascia in Dupuytren's disease should be removed. We recommend endoscopic operation, allowing us to achieve a successful outcome in 79% of cases.

Level of primary disability of adult population of the Amur region from 2005 to 2011 owing to eye diseases decreased with 6,00/000 to 1,40/000. In structure of disability the urban population prevails. In the Amur region, by data on the extremity of 2011, primary disability of the I group made 28%, the II groups - 25,8% and the III groups - 46,2% that is approximately comparable to distribution on the average across the Russian Federation.

Abstract
There were presented the results of the investigation of changes in the composition of microbiocenosis of the colon and the content of local metabolites (malondialdehyde, superoxide dismutase, catalase) in the correction of disbacteriosis caused by antibiotic with dry commercial and liquid copiright bifidumbacterin. It was noted that the simultaneous use of liquid bifidumbacterin with an antibiotic protects experimental animals from the development of disbacteriosis (the number of lacto- and bifidobacteria, Escherichia does not decrease, the amount of lactose-negative enterobacteria does not increase), whereas the use of dry bifidumbacterin helps to preserve only the number of bifidobacteria. When using the liquid form, the physiological activity of the local metabolic processes does not change, due to the balanced functioning of superoxide dismutase and catalase with the normal content of malondialdehyde. It was established that the corrective action of the liquid form of bifidumbacterin is more effective than the dry one.

Reological properties of nimodipin suppositories on poly (ethylene oxide) basis with 2% distillide monoglyceride depending on temperature conditions of suppository masses were studied. Abstract
The methods of limiting wetting angle and surface energy have been employed to detect the hydrolytic resistance in ceramic restoration systems - silicate feldspar baked ceramics; silicate glass-ceramics; oxide ceramics infiltrated with lanthanum aluminosilicate glass; polycrystalline oxide structural ceramics, and natural teeth. It has been shown that the synthesized polycrystalline oxide structural ceramics has prominent hydrolytic resistance, which provides it with adhesion resistance against the microbial deposits from the oral liquid. Reduced bacterial contamination at the prosthetic bed tissues helps improve the general hygiene in the oral cavity also adding stability to remote clinical outcomes thus working a positive effect on the efficiency of the restoration treatments.

Abstract
By means of clinical and laboratory methods the antioxidant system of unstimulated oral fluid is investigated in children aged 4,5 to 8 after the application of removable orthodontic appliances on the indices of diene conjugates, triene conjugates, Schiff bases, as well as the parameters of antioxidant activity. It was revealed that the base units of a photo curable composite «Versyo» due to the minimal growth of values of oxidant and antioxidant activity of mixed saliva without the development of «oxidative stress» provide the lowest possible adaptation periods in comparison with the appliances of the base plastics of hot and cold type of polymerization.

Abstract
The expanded bacteriological examination of urine, including isolation and identification of aerobic and non-clostridial anaerobic bacteria, was carried out in order to study the microbial spectrum of urine in patients with different forms of diabetes mellitus. In all cases, the mixed aerobic/anaerobic infection with the predominance of non-clostridial anaerobic bacteria (Peptococcus sp., Peptostreptococcus sp., Propionibacterium sp., Eubacterium sp.) and representatives of the gram-positive flora (Corynebacterium sp.) were found in urine. The general proven pathogens - E. coli - were isolated consistently more rarely. The data on the etiological structure should be taken into consideration for administration of the adequate antibiotic therapy.

Abstract
The analysis of the results of clinical-instrumental tests on 99 patients suffering from tumors or benign damages of proximal bile ducts complicated by obstructive jaundice compared to the control including patients without any cholagogic pathology has been carried out. The potentialities of ultrasonic diagnosis in detecting tumor affection of bile ducts at hilar cholangiocarcinoma as well as in differential diagnosing of tumor affection and benign damages of the liver and gallbladder have been studied. The echo-sign of pathological alteration of bile duct wall has been proposed. The frequency of the echo-sign in the clinical group compared to the control has been analyzed. We have come to the conclusion that detailed examination of bile duct walls and the lumen of common, left and right hepatic ducts should be included into diagnostic examination of a patient due to the fact that the proposed method can contribute to the revealing of hilar cholangiocarcinoma. This ultrasound symptom can be exploited in differential diagnosing of different portal fissure damages and in revealing of the type of tumor growth.

Abstract
The urgency of the issue of the prevention of acute postoperative pancreatitis (APP). The results of prophylaxis Nak in 159 patients with inhibitors exocrine pancreatic secretion (RV) sandostatin in combination with an antioxidant meksidol and 173 patients using pancreatic exocrine secretion inhibitor sandostatin. A significant difference in the incidence of OPP, depending on the method of prophylaxis. Of the 159 patients who received prophylaxis using an inhibitor of the exocrine glands (RV) sandostatin in combination with an antioxidant meksidol in 7 (4,40%) developed OPP. Of the 173 patients who received prophylaxis using an inhibitor of the exocrine glands (RV) sandostatin in 19 (10,98%) developed OPP. The combined use of Sandostatin with mexidol yielded a statistically significant improvement in the results of prevention OPP.

Abstract
Evaluation of the antihypertensive efficiency of 10 mg amlodipin rectal suppositories monotherapy of grade 2 arterial hypertension with brachiocephalic artery atherosclerosis in comparison with 10 mg amlodipin tablets during 4 weeks was carried out. It was established that amlodipin rectal suppositopies significant decrease systolic and diastolic arterial pressure and normalize its variability. Also linear rate asymmetry of carotid arterial blood flow essential decreases that indicates increase of brain perfusion. Rising of daily hypotensive activity uniformity after application of amlodipin rectal dosage form in comparison with tablet was determined. Side reactions and undesirable effects throughout amlodipin use for both dosage forms wasn't noted.

Analysis of the results provide adequate breathing in the surgical treatment of 50 patients with extensive purulent-inflammatory processes in the area of the head and neck. A study of heart rate variability (HRV) in different types of heliotherapy in the sanatorium treatment of adolescent boys with arrhythmias have a different level of physical development was conducted. It is established that the application of solar-air baths scattered radiation HRV positive trend is more pronounced than with the total radiation. Shifts HRV maximized in adolescents with low levels of physical development. The possible use of the data and the dynamics of HRV to optimize individual rehabilitation programs is discussed.

Thus, the main causes of «hand technique» anastomosis by invagination are technical defects of the intervention in telescopic anastomosis by invagination and the ischemic disorders in colorectal anastomosis by invagination. 48 patients were operated on the colon and rectum with «hand technique» anastomosis. Abstract
By means of neurohistological and histochemical methods of investigation we traced the morphological changes in the neurovascular components and phosphatases of the hard and soft palate mucosa of denture-carriers and individuals who did not use removable dentures constructions in a comparative aspect. Certain individual differences in neural structures were revealed, as well as peculiarities of the degree of nerve structures destructive changes, the esterases content, their topography, the number and the ratio of neurovascular complexes. Morphological changes in the vascular system, apparently, are caused by a constant compression of the prosthetic bed tissue formations with dentures.

Abstract
The paper describes the development of the local and general statement of free radical oxidation in patients with hemophthalmus during vitrectomy. We examined 71 patients who underwent surgical treatment of hemophthalmia. We observed a positive dynamics of free radical oxidation after the operation: a full recovery of biochemical parameters in the late postoperative period. Among the complications mentioned cataract development in some patients who did not receive antioxidant therapy. Vitrectomy improves the visual function and is supposed to be safe and efficient in patients with hemophthalmus.

Abstract
The aim of the study is to summarize our own research experience on interaction of respiratory system and blood circulation in patients with chronic obstructive lung disease (COLD). The main directions of the study include screening of combination of COLD and systemic arterial hypertension (SAH); pathogenesis of SAH and COLD; screening of the first dose effects of various antihypertensive drugs subject to their influence on respiratory function and a gas homeostasis. It has been revealed that prevalence of SAH in patients with COLD significantly exceeds the population rate. SAH is characterized by daily stability of arterial blood pressure, low night blood pressure reduction and high loading pressure in patients with COLD. Antihypertensive efficiency of the first doses of anthenol, indapamide, perindopril and telmisartan are comparable on obligation and blood pressure reduction rate. The first dose of the receptor antagonists to angiotensin II telmisartan provides a significant gain of peak expiratory flow, similar, but weaker effect is observed in response to indapamide intake.

Abstract
In vago-sympathetic trunks that are suitable to venous sinus frog heart, with preserved brain, in the high-frequency electric field, the luminescent centers were observed. In the intact nerve at the beginning of the venous sinus excitation there were two luminescent centers, and outside of the sinus venosus excitation (during ventricular systole) there was only one. At the peripheral end of the cut vago-sympathetic trunk - one center of a luminescence - afferent was observed. The area of afferent luminescent center was 38,7% less than the area of the afferent one. The area of the most intense zone of the efferent luminescent center was 50,5% less than the same area of the afferent luminescent center. The centers on intensity of a luminescence were broken by the computer program into 7 cuts. The distance between two luminescent center cuts is not different. This demonstrates that afferent and efferent signals connected with heart rhythm are transmitted to close situated nerve fibers.

Abstract
In this work the correlation analysis between indicators of antioxidant protection (on activity of superoxiddismutase) and a functional condition of vascular endothelium at patients with respiratory-cardiac pathology is carried out. For estimation of a condition of vascular endothelium during the laser Doppler flowmetry added with ionoforetic tests, we calculated an coefficient of endothelial function. It is proved that ischemic heart disease accession at patients with chronic obstructive pulmonary disease significantly reduces activity of superoxiddismutase. It is also established that low activity of antioxidant protection at patients with a chronic obstructive pulmonary disease promotes damage of vessels by free radicals, causes dysfunction of endothelium, loading to progress of atherosclerosis and development of ischemic heart disease.

Abstract
The content of endothelium-1 and C-type natriuretic peptide in the blood plasma in correlation with the gene polymorphism of endothelial nitric oxide synthase was studied in 35 patients with chronic obstructive pulmonary disease (COPD), 35 patients with arterial hypertension (AH) and in 40 patients with the diseases combination (COPD+AH). VNTR-polymorphism causes the hyperproduction of endothelium-1 and C-type natriuretic peptide in patients with a single disease and with the COPD+AH combination. The arterial hypertension conjunction in patients with COPD doesn't influence significantly the microvascular endothelium status, but the endothelial dysfunction presence in patients with COPD may be a precondition for the arterial hypertension progression.

Abstract
Experimental L-NAME-induced pre-eclampsia model in rats was achieved by L-NAME administration from 14 to 20 pregnancy days. Blood pressure elevation, gestational proteinuria, placenta microcirculation impairing, vascular tone dysregulation, and L-norvaline (10 mg/kg) and conventional medications (methyldopa 0,086 mg/kg/day & amlodipine 0,5 mg/kg) result in the evident correction of the pathological changes in pre-eclampsia model. Abstract
The aim of the work was to study the structure of periodontal disease in pregnant women in the Rostov region and ascertain impact on adherence to treatment for the disease. In the clinical group included 274 patients with periodontal disease: 107 patients with high and 167 patients with low adherence to treatment. Found that the prevalence of parodontal disease in pregnant women in the Rostov region is 68,8%. In the structure of parodontal diseases by frequency prevails hypertrophic gingivitis (69%), parodontitis occurs in 17,1%. The number of pregnant women with low adherence to treatment (60,9%) exceeds the number of patients who received the full course of treatment for parodontal pathology (39,1%). Commitment to pregnant women to treatment of periodontal disease is a key factor in ensuring a favourable dynamics of parodontal status.

Abstract
Conduct a study on the morpho-functional features of the pineal gland at the simulation laundry lighting and acute stress. It was determined that in the peripheral regions of the pineal gland conditions develop functional activity, as indicated by illumination of the region, as compared with the central part, by reducing the number of basophilic nuclei substance pinealocytes. These morphological features suggest a low functional activity of the pineal gland of rats under experimental conditions.

Abstract
100 patients with CHF FC III against the background of ischemic heart disease (IHD) and/or stage III hypertensive disease (HD) who received complex therapy (quinapril, torasemide, spironolactone) took part in the research. Upon randomization, Group 1 consisted of 56 patients (57,5±21,7 years old) and received metoprolol succinate of sustained release (betalok ZOK, Sweden’s, «AstraZeneca», 59,1±12,7 mg per day). Group 2 consisted of 44 patients (57,1±21,4 years old) and received Ivabradine, inhibitor of If channels in the sinoatrial node (coraxan, France’s, Servier, 12,1±4,6 mg per day) in cases when use of beta-blockers (BB) was not possible. VO2max treadmill tests, echocardiography, 24-hour blood pressure monitoring, NT-proBNP blood plasma test were done initially and 6 months in. A cardiorespiratory synchronism test was done in order to objectively evaluate the regulatory adaptive status (RAS). Complex therapy with the use of Ivabradine showed comparable clinical effects with metoprolol succinate: improved the structural and functional condition of the myocard, increased tolerance to physical exercise, caused positive shifts in the level of NT-proBNP in blood plasma and VO2мах during physical strain, but more than BB positively influenced the RAS. Therefore, Ivabradine can serve as an alternative to BB if it’s impossible to use in patients with CHF FC III against the background of IHD and/or stage III HD.

Abstract
The study involved 100 people (50 men and 50 women) aged 32 to 68 years. The first group consisted of 53 patients with chronic heart failure class II and III stage of hypertensive disease (29 men and 24 women, age 52,9±10,7 y. o.), treated with sustained-release metoprolol succinate (betaloc ZOK of «AstraZeneca», Sweden production, of 97,3±9,5 mg/day doze). The second group included 47 patients with chronic heart failure class II and III stage of hypertensive disease (21 men and 26 women, age 57,5±5,7 y. o.), receiving quinapril (accupro of «Pfizer», USA production in 24,7±13,4 mg daily doze. Treadmill activity with measuring of the maximum oxygen consumption on effort, 24-hour blood pressure monitoring, echocardiography and NT-proBNP testing, сardio-respiratory synchronism trial was carried out initially and after six months of therapy. Beta-blocker and angiotensine converting enzyme inhibitor are equally reducing blood pressure, are not identical in influencing the in statedly-adaptive status patients and the latter can be preferred.

The efficiency of an integrated comprehensive treatment including laser therapy in 50 patients with primary osteoarthritis. The article assesses the epidemiological situation related to the incidence of scabies in the Krasnodar region. The results of studies in six years. Observed decline in the incidence in the region and the high incidence of child population, mostly in the unorganized teams. Abstract
In article experience of treatment of patients with acute destructive pancreatitis (ADP), with monitor the severity of patients on a scale of SAPS and determination of concentration of a procalcitonin in blood. In depending on the parameters, patients received or a complex of conservative actions, with application is presented, in some cases, a diagnostic laparoscopy, or along with conservative therapy, operative measures were applied. At patients with the infected forms of ADP, operative measures depending on prevalence of pathological process, and as gravities of a condition of patients were applied. As a whole application of algorithm allowed to lower a lethality at ADP from 21,3% to 12,3%.

At present, in the structure of gynecological pathology the incidence of chronic inflammatory diseases of the reproductive system reaches 60-65%, and has no tendency to decrease. Abstract
The review article discusses recent advances in our understanding of viral myocarditis such as pathogenic role of infection and immune mechanisms in acute, sub-acute and chronic phases of the disease. A newly discovered Th17 lymphocyte subset and Interleukin 17 play an important role in the development of inflammatory myocardial diseases. The protective effect of Interferon y and Th1 immune response in acute myocarditis and the role of Th17-mediated mechanisms in progression of acute myocarditis to dilated cardiomyopathy are considered. The evidence of pathogenic role of Th17 lymphocytes in viral myocarditis in human is given.

Abstract
We report a case of primary diffuse meningeal melanomatosis, a rare variant of primary malignant melanoma of the CNS, in a 25-year-old woman. The disease was clinically manifested with obstructive hydrocephalus and severe polyneuritis of cranial and spinal nerves. The results of the CSF cytology and biopsy of the meninges were negative. There was a steady progression of the disease despite the success of the ventriculoatrial shunt. The disease was diagnosed only at autopsy.
